THC-O-acetate is a semi-synthetic acetate ester of THC that the body converts back into THC — a prodrug. It was marketed as “three times stronger” and “spiritual,” but the headline that actually matters is a safety one: heating THC-O can form ketene, a toxic lung gas, so it should never be vaped or smoked.

Why you must not vape it
Acetate esters of cannabinoids can pyrolyse to ketene when heated, a highly toxic gas associated with severe lung injury. Researchers have specifically flagged THC-O-acetate on this basis and drawn the parallel to the vitamin-E-acetate injuries of the 2019 EVALI outbreak. This is the single most important fact about THC-O: the delivery method, not the dose, is the danger.1
Prodrug behaviour and legality
Because it is a prodrug, THC-O effects come on slowly (30–60+ minutes) as the body cleaves the acetate to THC, which has led to accidental over-consumption. Legally it is a semi-synthetic in a shifting grey area; the US DEA has indicated THC-O-acetate is a controlled substance. In sport, only CBD is WADA-exempt.
