Nabilone is a synthetic analog of THC — a designed molecule, not identical to THC — that is FDA-approved as Cesamet for chemotherapy-induced nausea and vomiting that has failed to respond to standard anti-nausea drugs. Being synthetic and orally stable makes its dosing predictable.

Not the same molecule as THC
Unlike dronabinol (which is THC), nabilone is a structurally distinct synthetic that acts at the same CB1 receptor. It is reserved for chemotherapy nausea unresponsive to conventional treatment, and is also used off-label in some pain and spasticity settings.
